How do I create forms and collect e-signatures?

Create forms under Settings › Booking & policies › Forms & waivers from a template, a blank form or an upload of your paper form, which AllPaws turns into a draft for you to check. Choose who fills each one in and when, turn it on, and customers sign online or from a link you send.

Create a form

  1. Go to Settings › Booking & policies › Forms & waivers.
  2. Click New form (or Start from a template if you have no forms yet). Click a template to preview it, such as the Boarding/Daycare Contract or the Liability Waiver & Photo Release, then click Use template. Or click Start blank.
  3. To use your paper or PDF form, click Import your waiver and upload a PDF (up to 15 pages), JPEG or PNG of up to 10 MB. Save a Word file as a PDF first. AllPaws copies its wording and turns its blanks, signature lines and initials into fields, as a draft.
  4. In the editor, add blocks: text, questions, and sign-off blocks such as Signature, Initials and Agree checkbox.

Choose who fills it in and when

  1. Click Where this appears. Choose whether customers or staff fill it in, and when: when a customer creates an account or adds a pet, when they book the services you tick, before a booking is confirmed or checked in, at check-out (staff only), or only when you send it.
  2. In the same panel, under Frequency, choose Every time, just once, or once and then again every so many days, then click Done. A form asked for when someone creates an account is only asked once.
  3. If someone should look over each submission, click Staff review, tick Require review and click Done. If you pick reviewers there, only they can approve.
  4. Click Save changes, check the form with View as customer, then click Turn on.

New, template and imported forms start as drafts, so customers don’t see them until you turn them on. Changes to a live form stay hidden until you save.

Collect signatures

  • Customers fill in and sign forms on your booking page, at the points you chose. They draw their signature and type their initials.
  • To send a form for a booking, open the reservation’s Forms tab, click Share, then Send Email/Text or Copy link. To send any other form, such as one you only use when you send it, open the customer’s Forms tab and click Send Email/Text. Links don’t need a sign-in and work for 30 days; send the same form again within a week and it’s the same link. Texting a link needs the texting add-on.
  • A form with required questions that hasn’t been filled in stops the booking being confirmed, checked in or checked out, depending on where it appears. For a form customers fill in before check-in, you can tick Staff can waive this at check-in. Staff then waive it from the reservation’s Forms tab (Share › Waive form) and give a reason.
  • Forms waiting for review show in the bell and in Reports › Forms Report. Click Detail, then Approve.

Tips

  • Check Where this appears on imported and blank forms before you turn them on. A blank form starts out asked for on every booking.
  • A form that has been filled in can’t be deleted. Archive it instead, and its answers stay on file.
